This page is dedicated to those students that have made the decision to complete a Clinical Experience at Vanderbilt University Medical Center. This Clinical Experience could include VUMC research, shadowing, observational experiences or other opportunities.
As you are aware, you must meet VUMC Clinical Experience Immunization Compliance requirements.
Next Steps for this Clinical Experience Clearance
1. Complete this online survey/webform: redcap.link/studentclinicalexperiences
2. The Student Health Center will directly receive your submission. Your existing medical record will be reviewed to ensure you have met all VUMC requirements.
3. After the Student Health Review
a. If you are compliant with no issues or when you have cleared all your outstanding items, you will receive an email documenting your compliance. The clearance email from Student Health will be sent to the email that you provided to us in the survey.
b. If you do not meet compliance requirements, you will be contacted via My Health At Vanderbilt (MHAV) messaging with specific details regarding missing vaccinations, tuberculosis testing or other concerns. Student Health will send this via MHAV for patient privacy reasons. If you do not have an account in My Health At Vanderbilt, you will need to create one after completion of the survey. We will work with you to meet these requirements and eventually send the approval email noted above.
4. You will then provide this compliance approval email to your VUMC program. This is your responsibility.
Note that this online survey/webform is for VUMC Clinical Experiences ONLY. If you have a form regarding Immunization Compliance to be completed for any other institution, employer, etc. other than VUMC, call (615) 322-2427 and ask to schedule an appointment for a nurse visit for forms completion.
